Hey, how do I download it?
This is a warmer, and there is nothing to download. It's just an idea for your lesson, not a worksheet.
Personal information. This is a good activity to use at the beginning of the course or school year when, especially if the class is large, they may not know one another well. Draw this form on the board and tell the learners to copy it (you may change form if you want or add something ). Example: Name- Surname- Age- Adress- Date of birth- Place of birth- Hobby- Telephone number- etc. Ask two volunteers: Get one to ask questions and fill in the form on the board for the other. Let’s play: Collect in the form - put all their forms face down in a pile- pick one at random- get the class to identify the person described by asking : Example: -How old is she/he is? -male/female -What’s her/his name/adress etc. You may tell the class to work in pairs, asking and answering the questions and a) fillingin their own copies ; b) filling the form for each other. You may ask learners what they found out about their partners. Example: (Letters is the names of the learners) A, how old is B? B, what’s C adress? etc. You can make a file with all the forms and ask the class to revise it when it is necessary(for example to infer the date of the birth, to prepare a congratulations, or to know smb’s interests to make a usefull present.
